1. Open the App

Open eAuditor on your phone or tablet to create Food Allergy Form.

2. Create a Template

  • Go to Templates.

  • Tap Create Template.

  • Choose Blank Template.

3. Add Simple Sections for Food Allergy Form

Customer Information
Add fields for:

  • Customer Name

  • Contact Number

  • Date

Food Allergy Details
List common allergies. Use a checklist:

  • Peanuts

  • Milk

  • Eggs

  • Wheat

  • Soy

  • Shellfish

  • Tree Nuts

Add an Other” option so the customer can write another allergy.

Food Item Ordered
Write the name of the food or meal the customer orders.

Special Instructions
Write clear notes for the kitchen.
Example: Prepare food without peanuts.

Staff Confirmation
Ask the staff member to confirm that they read the allergy details.

Signatures
Collect signatures from the customer and the staff member.

4. Complete the Food Allergy Form

Open the template in eAuditor when a customer reports a food allergy.
Fill in the details and confirm the allergy with the customer.

5. Save the Record

Tap Complete Inspection.
Export the report as a PDF.
Save the file for food safety records.

This form helps staff record food allergies clearly and reduce the risk of allergic reactions.
